Trial description

Compare the proportion of patients for whom one or more supportive oncology care(s) (SOS) recommended in the personalized post-cancer care plan (PPAC) were implemented, based on a structured post-cancer pathway in a day hospital followed by nurse coordination, compared to standard care.
