Medical record

Dossier médical

The medical record is an essential tool for your entire care journey at Institut Curie.

Your computerized record contains administrative and medical elements.

Its role is to gather all the data about you for your medical monitoring: identity, state of health, test results, procedure reports, etc.

This information is intended only for the people involved in your care. All the recipients of such information are required to respect professional secrecy.

To access your record

According to the law, you can obtain your record either through a local doctor who you designate, by having it delivered by hand to Institut Curie, or by directly requesting that the document be sent to your home.

To request either certain elements of your medical record or of all of it, you must send a written request (attaching a photocopy of your identity card) to the Director of the Hospital Group of Institut Curie - 26 rue d'Ulm – 75248 Paris Cedex 05.

The forms are available in the "Useful documents" section.

Your right of access and rectification

Institut Curie is required to keep your medical record for a period of 20 years from your last treatment. No items may be deleted from this folder.

However, if you believe that the information contained in your medical record is incorrect, you can request that it be corrected by the medical team.

For any other requests, you can send a written request addressed to the Data Protection Officer (DPO) of Institut Curie at the following address: dpo@curie.fr.

The transmission of your personal data

For research purposes, under conditions of strict confidentiality and with the authorization of CNIL, Institut Curie may transmit data on your pathology to the cancer registry of your departement. According to the law, you may object to such transmission, and you have the right to access and rectify the transmitted information. You can contact the Director of the Hospital Group - Institut Curie - 26 rue d'Ulm - 75248 Paris CEDEX 05.
